Become a Patient

How to become a patient

Your insurance company may require that you get a referral from your Primary Care Physician before becoming a patient. Please check your policy. If required, you must get the proper authorization from your insurance company before scheduling your appointment.

Scheduling your first appointment

If a referral is required, the referring physician may schedule the appointment for you; or, once you receive the referral, you may schedule the appointment yourself by telephoning our office.
If your insurance does not require a referral, please call our office directly to schedule your first appointment or click the link below.
Appointment
When you come for your first appointment, please bring the following:
  •   Your insurance card
  •   A list of your current medications
  •   Your contact information (telephone and mailing address)
  •   Details of your medical history
  •   Your co-pay

If you would like to fill in your New Patient Registration forms before your first appointment, you may download them and fill them out ahead of time or submit the form online.
