Insurance

We accept most major insurances. If you have a question about whether or not your insurance is accepted, you may telephone our office prior to scheduling your appointment.

Cancellations

We require that you give us a minimum of 24-hour notice if you need to cancel or reschedule your appointment.

Arriving On Time

We value your time and we expect that you will value ours. Arriving on time is very important as late arrivals mean the person after you has to wait longer to be seen.

Because timeliness is so important to our ability to give good patient service to everyone, it is our policy that, if you are more than 10 minutes late, you will be given the option to wait to be seen or to reschedule for another day.

Arriving For Your First Appointment

For your first appointment, please plan to arrive 15 minutes early so that you may complete your paperwork. This paperwork must be done before you can see the healthcare provider. Please be prepared to provide information on your health, including your allergies, past and current medical conditions, current medications, etc.

You may also download the forms and complete them prior to your first visit, and then bring them with you to your first appointment.
